Immunizations and physicals necessary before school starts
School will be starting back soon and the Pendleton County school nurses would like to remind parents of the necessary health records required for the school year. All kindergarten students should have turned in an up-to-date Kentucky immunization certificate, an initial-entry physical, and an eye exam during the kindergarten registration process. If this has not been done, it should be completed upon the first day of school. The eye exam should be completed no later than January 1, 2009.
Students starting the sixth grade are required to have a new physical. (Sports physical is NOT acceptable.) and an up-to-date Kentucky immunization certificate. Children eleven to twelve years old are required to have a Tetanus booster. These records can be turned in to the school nurse at sixth grade orientation prior to the start of school or on the first day of school.
The necessary physicals and immunizations for school enrollment are available at your doctor's office or the local health department. Please arrange for your child to have his/her physical and immunizations for a healthy 2008-2009 school year.
