David Paul Fields, 41, of Berry passed away Friday, July 6, 2012, at his home.

Surviving are wife; Cynthia LeAnn Salyers Fields; children, Mackenzie Jordan Fields, Jacob David Fields, Walker Lee Fields, Cynthia Grace Ann Fields; and  step-mother, Sandy Fields.

Interment will be held at 4 p.m. on Tuesday, July 10, 2012, at Pythian Grove Cemetery in Berry.

Memorial service will be held from 6-8 p.m., Tuesday, July 10,  at First Love Church (Outlet Mall) Dry Ridge.
